In appreciation for all the hard work you are doing to sell our product, for new groups choosing the Community Care Network (CCN) Plans M, N, X & Y, effective the fourth quarter of 2016, Affiliated Physicians & Employers Health Plan (APEHP) is offering a Broker Bonus of $45 per enrolled employee.

Disclosures

Only new groups effective fouth quarter 2016 are eligible to receive bonus.
Bonus is a one-time payout, paid in March 2017 commission statement.
Must be a APEHP appointed producer.
Bonus paid out on new business only with at least one employee choosing Plan M, N, X or Y.
Bonus paid out on entire group as long as one employee enrolles in Plan M, N, X or Y.
Minimum 10 enrolled employees to qualify - can be spread over multiple groups.
