Whenever possible, we appreciate having submissions at least 2 weeks prior to
the requested effective date.
Below are cutoff dates for submitting cases to our office.
Cases received after these dates might not receive the requested effective
date. If you are unable to meet these dates, please contact your account
team representative as soon as possible.
